LANDLORD PROGRAM FORMS

Owner Information Packet This is information about the Housing Choice Voucher program.
Inspection Checklist Use this form to conduct a pre-inspection of your unit so that you can identify and correct any potential fail items.
Lease Requirement Use this form to ensure the Housing Authority has all the information needed to complete contracts and issue payments promptly
Sample Housing Assistance Payment Contract This is a sample of the contract the owner signs with the Housing Authority.
Tenancy Addendum In the Housing Choice Voucher program, owners may use their own lease with this addendum.
Request to Release Tenant Information This allows us to release information about a tenant to a prospective landlord
Request for Rent Increase/Change of Terms Use this form to request a rent adjustment. Please note that the Housing Authority must approve the total amount of rent that you collect from both the tenant and the Housing Authority.
Change of Ownershp and HAP Contract Transfer Form Use this form to report a change in ownership of your unit.
Authorization to Act on Owner’s Behalf Use this to authorize someone else (such as a property manager) to act on your behalf.
Landlord Direct Deposit Form Use this form to set up direct deposit and have the Housing Authority deposit payments to your account.
Landlord Direct Deposit Cancelation and/or Change Use this form to cancel and/or change direct deposit information.
Notice of Occupancy Rights under the Violence Against Women Act The Violence Against Women Act (VAWA) provides protections for victims of domestic violence, dating violence, sexual assault, or stalking. VAWA protections are not only available to women, but are available equally to all individuals regardless of sex, gender identity, or sexual orientation. You can fill out this form to show that you are or have been a victim of domestic violence, dating violence, sexual assault, or stalking, and that you wish to use your rights under VAWA.
Certification of Domestic Violence, Dating Violence, Sexual Assault, or Stalking, and Alternate Documentation The Violence Against Women Act (“VAWA”) protects applicants, tenants, and program participants in certain HUD programs from being evicted, denied housing assistance, or terminated from housing assistance based on acts of domestic violence, dating violence, sexual assault, or stalking against them. Despite the name of this law, VAWA protection is available to victims of domestic violence, dating violence, sexual assault, and stalking, regardless of sex, gender identity, or sexual orientation
